Information you choose to provide

The contact form may ask for a name, email address, phone number and a short message. That information is routed through Peacemaker's GoHighLevel/LeadConnector contact system so the care team can respond.

How contact information may be used

• Respond to the inquiry
• Clarify the requested service and schedule
• Coordinate a consultation
• Maintain the business record needed to manage the request
• Send marketing or promotional communications only according to the consent choices shown on the form

Minimum-necessary information

Use the form for basic contact, location and scheduling information. Do not include a diagnosis, assessment, medication list, medical record, detailed care schedule, insurance document or another person's private information.

Optional website analytics

Google Analytics loads only after you choose Allow analytics. If allowed, it may process the page path, referring page, general device and browser information, approximate location, and whether a call, contact or Microsoft Bookings link was selected. Peacemaker does not intentionally send names, phone numbers, email addresses, form contents, diagnoses, care needs, insurance documents or other sensitive details to Google Analytics. You may decline analytics or change your choice at any time through the Analytics preferences link in the site footer.

Service providers and links

The site uses third-party services for the contact form and Microsoft Bookings. Those services process information under their own terms and privacy practices. External links may lead to websites Peacemaker does not control.

Questions, access or deletion requests

Call Peacemaker at (502) 572-7377 or use the contact page without including additional sensitive details. Some records may need to be retained for legal or operational reasons.
