Information you choose to provide

The contact form may ask for a name, email address, phone number and a short message. That information is routed through Peacemaker's GoHighLevel/LeadConnector contact system so the care team can respond.

How contact information may be used

• Respond to the inquiry
• Clarify the requested service and schedule
• Coordinate a consultation
• Maintain the business record needed to manage the request
• Send marketing or promotional communications only according to the consent choices shown on the form

Minimum-necessary information

Use the form for basic contact, location and scheduling information. Do not include a diagnosis, assessment, medication list, medical record, detailed care schedule, insurance document or another person's private information.

SMS consent and your choices

If you choose to receive text messages from Peacemaker Elder Care, we use your phone number and record of consent to send only the message categories you select. The contact form offers separate, optional choices for marketing messages about our non-medical home care services and offers, and for non-marketing consultation and appointment confirmations, reminders and rescheduling messages. You can submit an inquiry without agreeing to either SMS category. Being in our contact database does not by itself enroll you in text messages.

Mobile information and SMS consent

We do not sell or share mobile information with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent are not shared with third parties for their own use. Service providers may process this information only on our behalf to operate and deliver the messaging service.

Data security and access to contact records

Peacemaker restricts access to its GoHighLevel contact and SMS consent records to its authorized business development representatives and marketing representatives. Access to GoHighLevel is granted only to those representatives. Please use the general contact form only for basic contact and scheduling information; do not send sensitive health information.

Optional website analytics

Google Analytics loads only after you choose Allow analytics. If allowed, it may process the page path, a privacy-limited referring source, general device and browser information, approximate location, and whether a call, contact, Microsoft Bookings or downloadable guide link was selected. For the Louisville Facebook community-guide pilot, the site accepts only the fixed Facebook source and campaign labels on the two approved guide pages, derives the guide identifier from the page, removes the full query string before Analytics loads, and records an attributed guide-page view or matching direct-PDF selection. It does not send a Facebook group, administrator, member, representative, PDF URL, form value, or anything written in a saved or printed guide. The PDFs remain direct and do not require a form or contact information. Peacemaker does not intentionally send names, phone numbers, email addresses, form contents, diagnoses, care needs, insurance documents or other sensitive details to Google Analytics. You may decline analytics or change your choice at any time through the Analytics preferences link in the site footer.

Optional advertising measurement on getcare

On getcare.peacemakereldercare.com, the Google Ads tag loads only after you choose Allow advertising measurement. If allowed, it may process limited browser, device, page and advertising-click information and may record when you select Peacemaker's published phone link or when the embedded LeadConnector form reports a successful submission. A phone-link selection shows intent; it does not confirm that a call connected. Peacemaker's conversion command contains only the Google Ads action destination and does not intentionally include names, email addresses, phone numbers, form contents, diagnoses, care needs or other sensitive details. Ad-personalization signals are disabled. Peacemaker uses aggregate conversion reporting to evaluate advertising performance. This choice is separate from optional Google Analytics on the main website, and you may decline or change it through the Advertising measurement preferences link on the getcare site. The LeadConnector form is a separate third-party service and loads whether or not you allow Google Ads measurement; information entered in that form is processed under LeadConnector's own terms and privacy practices.

Questions, access or deletion requests

Call Peacemaker at (502) 572-7377 or use the contact page without including additional sensitive details. Some records may need to be retained for legal or operational reasons.
